The initial phase of hospice care involves an assessment process. In this phase, a physician assesses the patient’s health status to decide if they qualify for services within the hospice program.
Once confirmed as eligible, the patient will be entrusted to a multidisciplinary team that includes a medical director, nurses, social workers, home health aides, chaplains, volunteers, and other experts skilled in end-of-life care. This team will devise a customized plan tailored to the patient’s specific needs.
